Family Reflections is a process I developed to get honest and helpful feedback from families. In my experience, parents often will tell me of a suggestion or issue in the classroom on a form that I might not otherwise hear. I think this is because a form is easy and quick!
While I send home about four to five reflection forms a year, today I am sharing the last Family Reflection form of the year. I keep it to one page – short and sweet. I do not want the form to be overwhelming. Click here to download a copy.
I did something like this a few years ago, when I looped and knew I would be having the same group of kiddos.

I liked the form.
I picked up some of the ideas for my school and families.
I was wondering for further questions for me to get feedback. And there they are.
Thank you

Hi again, I came back to your blog.
Evidently no obligation, but if you wish, you can have a peek at my blog, where I say things about family, as concerning with education. You can click on Label “Family”.
Hope it be any useful.
Feel free.

I was unable to read all of the reflection form, is there a way I can get a clear copy?
Sorry about that. I’m seeing if I have a clearer copy to upload here.
Hi Tunise, thanks for your patience. I’ve recreated the form as a .pdf you can download and print. There’s a link in the blog post you can click to download it for free. Thank you! – Sally
You mentioned that you send home four or five reflection forms to families each year. I’m curious…what are some of the topics/purposes of those? Thank you!
I send home reflection forms after field trips, special projects and investigations. Anything we are doing as a class that I want to see how the children express their involvement to the parents and how the parents felt about the experience.
